The external problem is what the protagonist has to face because of that internal conflict. Basically, in a Person vs. Self conflict, the internal conflict is the reason there’s an external problem that needs solving, and solving that problem becomes the plot.

That’s why a Person vs. Self conflict typically has representatives of the protagonist’s problem that work as antagonists. The problem might be the addiction, but people are involved somehow in dealing with that addiction. Think of it as a symbolic antagonist.

For example:

The addicted protagonist might have a best friend who drags her out to the club scene every night.

The protagonist who fears commitment might have an unhappy sister in a miserable marriage who constantly tells her, “Never get married.”

The arrogant protagonist might have a husband who feeds her ego and tells her she’s doing the right thing for her career, even when she’s failing.
